Ascension Catholic Academy welcomes all stock and mutual fund transfers, with no minimum requirements. Stock brokers require instructions in writing. Our broker is UBS Financial Services, and stock is received by the Church of the Ascension. You will need to provide one copy of this form to your stock broker, and mail or email one copy to us at Ascension (address or email provided on the form). It is important to notify Ascension Catholic Academy when you have completed your request to your broker to ensure that your gift is properly credited.
